Education Access Cablecast Request Form
Fill out an Education Cablecast Request Form (CRF) for SWTV 14 by clicking the above link. Paper CRFs are still available in the Producers' Self Service Area at Edina City Hall or can be requested via e-mailing the Community Television Administrator at dnissen@EdinaMN.gov.
Public Access Cablecast Request Form
Fill out a Public Access Cablecast Request Form (CRF) for SWTV 15 by clicking the above link. Paper CRFs are still available in the Producers' Self Service Area at Edina City Hall or can be requested via e-mailing the Community Television Administrator at dnissen@EdinaMN.gov.
Outside Production Tracking Form
Public Access Producers who do not reside in Eden Prairie, Edina, Hopkins, Minnetonka or Richfield must acquire a program sponsor who is a resident in one of the five member cities of the Southwest Suburbs to broadcast a program on SWTV Channel 15.
SWTV Media Release Form
Filling out an SWTV Media Release Form allows Southwest Community Television to use photos, videos, and other media depicting the submitter(s) on its website, channel(s), and in social media. A printed form can be signed and faxed to ATTN: Community Television Admin at 952-826-0389 or mailed to 4801 W. 50th St. Edina, MN, 55424. This form can also be downloaded, filled out electronically, and emailed to dnissen@EdinaMN.gov.
Submitters will be required to fill out a Media Release Form every six months.
The preceding are working documents subject to change pending evaluation by the Southwest Suburban Cable Commission.
Hours for Media Drop-Off
Hours for drop-off are 7:30 a.m. to 8 p.m. Monday through Friday, except holidays. All media should be brought to Edina City Hall, 4801 W. 50th St., and left in the producers’
self-service area outside of the Communications & Technology Services Department on the first floor. Producers dropping off media after 4:30 p.m. can only access the building via the entrance off of Eden Avenue.
